Terms and Conditions
Cancellation Period
It is the duty of the patient to provide PhyzioFit with a minimum of 24 hour notice to cancel or change appointments. Should a patient be late for an appointment, without complying to the 24 hour notice policy, the therapist has the right to alter the length/cancel the massage and the full price of the booked service will have to be paid. Patients will have a 20 minute grace period before their booking is terminated.

Contraindications
Contraindications are specific conditions which will prohibit the use of PhyzioFit services. The list of contraindications is as follows: Arthritis, Cancer/Undiagnosed lumps or bumps, Cardiovascular Conditions, Diarrhoea and Vomiting, DVT, Haematoma, Haemophilia, Hernia, Inflamed/Trapped Nerve, Nervous System Dysfunction, Osteoporosis, Pregnancy, Recent Fracture (<3 months), Scar Tissue (<2 years post major operation, <6 months post minor) and Skin Abnormalities (Abrasions, Bruises, Cuts, Sunburn, Etc.) localised to the area of the body requiring PhyzioFit services. It is the responsibility of the patient to identify contraindications/contact PhyzioFit via enquiries@phyziofit.co.uk to determine suitability for PhyzioFit services. If any contraindication is identified, patients are requested to not book PhyzioFit services. By proceeding to book a service, and should there be no prior enquiry, it shall be assumed that the patient does not have any contraindications nor any reservations/queries regarding them.

Booking Fee
Booking fees made via the official PhyzioFit website will be deducted from the total cost. Payments made on the day of the consultation will cover any remaining fees and can be made via cash, credit or debit cards. Due to the nature of the services provided, all payments, including booking fees and in-person payments, are non-refundable. Single/Block booking is not confirmed until the deposit payment is received. Failure to make payments without notification to PhyzioFit may result in further collection action. PhyzioFit will have full discretion for unpaid accounts and will take necessary action to collect any unpaid balances.

Booking (Blocked Appointments)
In order to block book appointments, dates must be specified and agreed by both the payee and the therapist. Appointments can only occur on the specified dates but can be used by friends/family of the payee. If the payee wishes to gift any number of booked appointments to friends/family, PhyzioFit must be notified a minimum of 1 week before the appointment is due. PhyzioFit must also receive written consent, from the payee, stating which specific appointment is to be gifted away and which individual shall receive the appointment. Each appointment is one-person use only.

The individual who is attending the appointment must be legible for the services provided and must not suffer from any of the following contraindications: Arthritis, Cancer/Undiagnosed lumps or bumps, Cardiovascular Conditions, Diarrhoea and Vomiting, DVT, Haematoma, Haemophilia, Hernia, Inflamed/Trapped Nerve, Nervous System Dysfunction, Osteoporosis, Pregnancy, Recent Fracture (<3 months), Scar Tissue (<2 years post major operation, <6 months post minor) and Skin Abnormalities (Abrasions, Bruises, Cuts, Sunburn, Etc.) localised to the area of the body requiring PhyzioFit services. A health declaration form must be completed by the attending individual prior to every appointment within the block.

To allow for suitable adjustments, the payee must notify PhyzioFit, a minimum of 48 hours prior to the appointment, about any cancellations/issues regarding appointment dates. It is the duty of the payee to notify PhyzioFit of any issue regardless of who shall be attending the appointment. If no prior notification is given, non-attendance of any appointment will result in the appointment being deducted from the block. Refunds or alternate appointments will not be issued for any missed appointments within the block. Should the payee request a further appointment after non-attendance, full payment in advance will be required.

Pain/Discomfort
Patients may feel pain/discomfort during the massage. If the massage becomes unbearable, it is the duty of the patient to inform the therapist so that techniques may be adjusted to their level of comfort. It is very common to experience pain, bruising and soreness post-massage, as it is part of the healing process. PhyzioFit is not liable for any injuries/issues that may occur post-massage.
Parental Guidance
Clients under the age of 18 years old must be accompanied by a parent/guardian. The accompanying parent/guardian must consent to the service, co-sign the Health Declaration Form and remain present throughout the treatment.

Pre-Consultation
Prior to the consultation, patients will be provided a Health Declaration Form which must be completed accurately and truthfully. In accordance to GDPR, all information provided will not be shared and will only be accessed by licensed PhyzioFit staff members. It is important that all information on the declaration form is correct and up to date, and patients should notify therapists immediately should this information change. Phyziofit is not liable for any incorrect information supplied and any injuries patients may suffer as a result of misinformation.

Refund Policy
Due to the nature of the services provided, refunds will not issued under any circumstances.

Service Termination
Services will be terminated if a patient demonstrates physical, verbal or sexually inappropriate behaviour and any payments made will not be reunded. Behaviour as such can/will result in a temporary/permanent ban, and may also result in legal action. Due to the close contact nature of the services, therapists will maintain a high standard of personal hygiene which patients must also follow. Bookings can/will be cancelled if patients exhibit poor hygiene standards.
